DVA forms for compensation cause unnecessary suffering

The form seems straightforward enough. The user fills in fields on a web page, and once they’ve filled in every field they can open the next page.

What could go wrong with that?

The form is the Department of Veterans’ Affairs (DVA) application for compensation resulting from service-related illness or injury, and the person who witnessed someone attacking a computer is a veteran’s advocate.
